Custom orthotics are very cheap and easy to make. Physical therapists, chiropractors, and podiatrists are trained in how to make them; however, most are not competent about best practices for prescribing them.

Typically, a clinic owner will instruct employees to recommend custom foot orthotics for as many patients as possible. Because, as the saying goes, it’s money in the bank. Clinics can charge as much as $200 to $800 per pair of custom orthotics which seems ridiculous. They take only about 20 minutes to evaluate for and cast. Do the simple math. Labor for 20 minutes of work = $7.20 + materials (plaster, plastic, shipping materials & costs) = $20 + labor for fabrication = $15. Total Costs to business owner = $42.2 – $50. This makes between $150 – $650 profit.

Dr. William O. Roberts, a sports medicine physician in St. Paul, Minnesota notes that “If your main business is feet, and part of your income is prescribing orthotics, than you might prescribe them 90, 100 percent of the time. It’s a financial issue, and I don’t think there’s a huge need for custom orthotics.”
